What a person asked for, held where the next service can read it.

The Mental Health Act 2025 introduces statutory Care and Treatment Plans and Advance Choice Documents, and places a duty on services to give a reason where a clinician decides not to follow a person's advance choice. Accord holds those documents portably and captures the reason at the point of departure.

Statement

What the person asked for, in their words, with the date it was recorded and who witnessed it.

Standing

accord.standing

Whether the statement is current, superseded, or withdrawn, and by whom.

Event

accord.event

A decision taken by a service that touched the statement.

Departure

accord.departure

Where the decision did not follow the statement: the reason given, the named person who gave it, and the time.

Seal

accord.seal

The record closes to further edit. Corrections are appended, never overwritten.

What we are waiting for

Mental Health Act 2025 Code of Practice

The consultation on the revised Code has not concluded. The Code will state what a service must record when a clinician decides not to follow an advance choice, and in what form. Accord's departure record is designed against the Act, and its field set is not fixed until the Code is published.

Commencement regulations

Statutory Care and Treatment Plans and Advance Choice Documents commence in phases. Until the sequence is confirmed, Accord is not deployed with a customer.

Loric Accord is a record-keeping tool. It holds what a person asked for and what a service did, and does not diagnose, treat, prevent or monitor any condition, does not assign clinical priority, and is not a medical device.
